Causes of Chest Pain
Many individuals experience chest pain at some point in their life.
The challenge, however, is figuring out the cause as many conditions can lead to chest pain.
Symptoms may vary among males and females, and the location of the pain may differ as well (left sided, right sided, middle of the chest, etc.).
No matter the presentation of chest pain, the life-threatening or emergent causes should be considered.
But how can you remember what those emergent causes are?
Read on to find out!
This lecture will provide you with a simple trick to remember the main life-threatening and emergent causes of chest pain!
The 4-2-1 Rule
We came up with a simple trick to help you remember the main life-threatening causes of chest pain.
We call it “The 4-2-1 Rule”.
Here is how it works…
It is called the 4-2-1 rule because there are:
4 heart related emergencies that can cause chest pain
2 lung related emergencies that can cause chest pain
1 esophageal related emergency that can cause chest pain
Now for the cool part!
You can remember there are 4 heart related emergent causes because there are 4 chambers in the heart.
4 heart related emergencies = 4 chambers in the heart
You can remember there are 2 lung related emergent causes because there are 2 lungs in the body.
2 lung related emergencies = 2 lungs in the body
And you can remember there is 1 esophageal related cause because there is 1 esophagus in the body.
1 esophageal related emergency = 1 esophagus in the body
Let’s go through the 4-2-1 rule below!
**The 4-2-1 rule provides an easy way to remember some of the main emergencies that can cause chest pain. It is not a complete list and other less urgent causes can become emergent.
4 Heart Related Causes
Using the 4-2-1 rule, we said there are 4 heart related emergencies that can cause chest pain.
They include:
Acute Coronary Syndrome (ACS)
Aortic Dissection
Pericarditis/Myocarditis
Pericardial Effusion/Cardiac Tamponade
1. Acute Coronary Syndrome (ACS)
Acute coronary syndrome is an umbrella term for the conditions involving acute myocardial ischemia or infarction (decreased blood flow to the heart).
ACS includes conditions such as ST-elevation myocardial infarction (STEMI), non-ST-elevation myocardial infarction (NSTEMI), and unstable angina.
2. Aortic Dissection
Aortic dissection is a tear to the innermost layer of the aorta.
3. Pericarditis/Myocarditis
Pericarditis is inflammation of the pericardium, which is the membrane sac around the heart.
Myocarditis is inflammation of the heart muscle.
4. Pericardial Effusion/Cardiac Tamponade
Pericardial effusion is the accumulation of extra fluid in the pericardium around the heart, specifically in the pericardial cavity or space.
Cardiac tamponade is compression of the heart from too much fluid build up around the heart.
2 Lung Related Causes
Using the 4-2-1 rule, we said there are 2 lung related emergencies that can cause chest pain.
They include:
Pulmonary Embolism (PE)
Pneumothorax
1. Pulmonary Embolism (PE)
A pulmonary embolism is a blockage in the arteries in the lungs.
2. Pneumothorax
A pneumothorax is a collapsed lung.
A pneumothorax occurs when air or gas accumulates in the pleural cavity or space between the lung and chest wall.
1 Esophageal Related Cause
Using the 4-2-1 rule, we said there is 1 esophageal related emergency that can cause chest pain.
This includes:
Esophageal Perforation/Rupture
1. Esophageal Perforation/Rupture
An esophageal perforation or rupture is a tear or hole in the esophagus.
This can lead to food and fluid leaking out of the esophagus.
Bringing it Together…
The 4-2-1 rule can be used to help you remember some of the life-threatening or emergent causes of chest pain.
The 4-2-1 rule includes 4 heart related emergencies, 2 lung related emergencies, and 1 esophageal related emergency.
Heart
Acute Coronary Syndrome (ACS)
Aortic Dissection
Pericarditis/Myocarditis
Pericardial Effusion/Cardiac Tamponade
Lungs
Pulmonary Embolism (PE)
Pneumothorax
Esophagus
Esophageal Perforation/Rupture
The “BREATHE” Mnemonic
Now for another cool trick!
You can take the 4-2-1 rule to remember the main emergent causes of chest pain, and combine that with the mnemonic “BREATHE”.
We came up with the mnemonic “BREATHE” in a previous lecture to help us remember the main emergent causes of shortness of breath.

So you can see how the “BREATHE” mnemonic along with the 4-2-1 rule provides a great way to remember some of the main emergent causes of chest pain and shortness of breath.
**The 4-2-1 rule and “BREATHE” mnemonic are not complete lists of every cause of chest pain and shortness of breath, and there are other causes that can become emergent too.
Causes of Chest Pain
Below is a list of other conditions that can cause chest pain.
It is difficult to remember them all, so having a simple way to remember some of the emergent causes using the 4-2-1 rule is very helpful.
